KEY BENEFITS OF THIS APPROACH

The switch to outcomes-based contracting has enabled a much more collaborative, flexible approach to problem-solving. This has facilitated a number of significant delivery innovations, including:​

  • Personalisation funds that Delivery Partners can spend on whatever is needed to support their service users to improve their wellbeing and achieve outcomes.

  • Working with the local CCG and primary care services (for health-related programmes) to facilitate the referral process.

  • An asset-based approach, whereby participants are supported to build on their own interests and hobbies.

  • Investing in the delivery teams so they are properly structured to service users' needs (from dedicated referral teams to ETE support). ​

IN FOCUS: IMPROVING THE LIVES OF ADULTS WITH LONG-TERM CONDITIONS

Ways to Wellness is an innovative programme for people with long-term health conditions (LTCs) based in Newcastle upon Tyne. The service is designed to improve the quality of life of those with LTCs by providing community health interventions that create sustained lifestyle changes and improved self-care.

 

By helping their service users manage their daily lives in a healthier, more sustainable way, Ways to Wellness complements patients's existing medical support, while trying to reduce their use of mainstream health services. It helps them feel more confident to manage their long-term conditions and make positive lifestyle choices.

This 'social prescribing' service involves care and management of the body and of the LTC, adapting everyday activities and roles to the condition and dealing with the associated emotions. 

"If you're retired like me, loneliness is the killer. You wake up in the morning and you have nowhere to go. Medically we go to the doctor - but not everything is necessarily solved by doctors. The social aspect has to be looked at differently. So Ways to Wellness has helped me a lot."
