This has been achieved  through a commitment of all stakeholders to continuously adapt the service to the users’ needs, that ultimately drives participants outcomes. This approach, has generated a number of initiatives that have enhance performance, the most relevant ones are:

  • Personalisation funds for Delivery Partners to spend on whatever is needed to support their service users to improve their wellbeing and achieve outcomes.

  • Invest in additional support  such a a mental health nurse was hired to accelerate the diagnosis and referral to mental health / drug and alcohol support services and to provide a more flexible and integrated approach.

  • Asset based approach, participants are supported to build on their own interests and hobbies 

  • Development of a unique partnership, bringing together specialist homelessness delivery partners with housing providers to maximise the opportunity of long term impact for rough sleepers

That means providing the intensive emotional and practical support they need to maintain successful tenancies, and to access appropriate health, training and employment services.

GM Homes aims to tackle long term homelessness through new and different approaches

 

GM Homes aims to provide the right homes, in the right places, at the right times. But also to offer flexible, local, long-term support to those we work with – recognising that different people might need different support at different times to meet their particular needs. 

 

GM Homes works with entrenched rough sleepers over a three-year period to get them off the streets and help them build a new life. That means providing the intensive emotional and practical support they need to maintain successful tenancies, and to access appropriate health, training and employment services.

 

GMHP is a unique collaboration between local housing providers, social investors and three leading homelessness charities. It is part of a wider strategy for tackling homelessness in the region, which includes programmes designed to prevent people become homeless and to provide support quickly if they do become homeless.
